Coronary bifurcations (BIFs) remain one of the most fascinating and challenging lesion subsets in interventional cardiology, with a lower procedural success rate and increased rates of long-term adverse cardiac events (1). Despite great in- terest in this complex lesion subset, treatment of BIF is still a controversial subject and multiple technical strategies have been proposed. We aim to present an overview of BIF anatomy, physiology, and classifi- cations and evaluation of the most recent interven- tional strategies. We highlight the most debated issues and propose our recommendations for an optimal and relatively simple approach while empha- sizing bench-testing results.
