When treating chronic total occlusion (CTO), the wire tip must be directed to the distal true lumen; therefore, visu- alization of the distal lumen is essential. A 77-year-old woman with effort angina underwent coronary angiography that revealed CTO of the proximal right coronary artery (RCA).
